Registration

AYSO REGISTRATION 2013

The early registration period has ended. However we have extended registration to June 15th.  Registration cost is now $120.  Only mail-in registration is open at this point.

Step #1: Complete and print registration forms by visiting to www.eayso.org. (Hand written forms will not be accepted. New players will need to provide a copy of their Birth Certificate.)

Step #2: Mail completed registration form and payment to:

**New players need to include a copy of their BIRTH CERTIFICATE. New players cannot be registered without a copy of their Birth Certificate.

Forms Postmarked after June 3, 2013 will be put on waiting list.
